Follow Up Consultation

45 minutes @ $100.00 Online/video chat

Follow up Functional Medicine visits include a 45 minute virtual visit to review laboratory results, follow up on treatment plans, discuss your progress, develop new goals, etc... Prior to this visit you will complete a Follow-up Visit update form that you will receive via email. 

This rate does not include any diagnostics or treatment plan that may be recommended during our visit. This also includes the time I will spend (30-60 minutes) thoroughly reviewing your Visit Update form, new lab results, diagnostic results, prior visit notes, and developing a treatment plan prior to your follow-up visit.

This also includes my time writing a thorough collaborative plan we have developed during our visit together (generally up to one hour after the initial visit writing up a clear follow-up plan with detailed information on all topics discussed).
